Membership can be cancelled at any time.
All requests to cancel must be made in writing via fax, e-mail or regular mail. If a member cancels within thirty (30) days of the enrollment date (within 30 days of receipt of membership materials for residents of CO, IN, MO, MT, ND, OH, OK, SC, and SD), you will receive a full refund. (The application fee, if required, is not refundable except in AR, CO, MD, and TN.) UHP reserves the right to terminate a program member from any of the discount programs for any reason including non-payment. In this event program fees will be prorated and refunded in the same manner as noted above. (Refund policy pertains only to customers purchasing the Premium Savings Plan. Does not apply to the premium RX Plan)
